Terms and Conditions of Service

1. Patient Privacy and Confidentiality

a.) Find My Focus strictly adheres to the Health Insurance Portability and Accountability Act (HIPAA) and all relevant privacy regulations to protect patient data.

b.) All patient health information (PHI) is securely stored and accessed only by authorized personnel with a legitimate need.

c.) Communications are encrypted and transmitted over HIPAA-compliant platforms. Patients are encouraged to avoid using public or unsecured Wi-Fi when engaging in telehealth sessions.
 

2. Telehealth and Technology Use

a.) All consultations are delivered via telehealth, using secure, HIPAA-compliant video platforms such as Zoom for Healthcare.

b.) As part of our standard care protocol, a licensed medical assistant or clinical support team member will be present during each session for documentation and care coordination purposes. Patients will be notified of their presence. Participation in a telehealth session constitutes consent to this model.

c.) Patients are responsible for ensuring a quiet, private environment during sessions. Sessions may be rescheduled if the setting is not appropriate for clinical care.

d.) Appointments must not be attended while driving or operating machinery. Doing so may result in session termination.
 

3. Scope of Services

a.) Find My Focus provides assessment, diagnosis, medication management, and care coordination for ADHD and related conditions through telehealth.

b.) We focus on evidence-based care delivered with compassion and respect, honoring each patient’s unique background, including faith and family dynamics where appropriate.

c.) Services are limited to patients physically located in states where our providers are licensed to practice.
 

4. Medication and Prescription Policy

a.) Medications, including extended-release stimulants, are prescribed only after a thorough telehealth evaluation and according to state and federal law.

b.) We do not routinely prescribe benzodiazepines or other controlled substances outside ADHD treatment protocols.

c.) Patients must attend regular follow-up appointments to continue receiving prescriptions. Missed appointments or lack of communication may result in a pause in medication management.

d.) All prescriptions are sent electronically to the patient's pharmacy of choice. It is the patient’s responsibility to ensure pharmacy details are up to date.
 

5. Billing and Payment

a.) Find My Focus bills insurance plans where applicable. Patients are responsible for all charges not covered by insurance.

b.) Accurate and current insurance information must be on file. Failure to provide valid coverage may result in suspension of services until resolved.

c.) Payment plans may be available upon request. Outstanding balances must be paid prior to scheduling follow-up care.
 

6. Appointment Policies

a.) Appointments must be scheduled in advance. Reminders will be sent via email or text.

b.) Late arrivals, cancellations without notice, or repeated no-shows may result in additional fees or discharge from the clinic.

c.) Patients are responsible for notifying staff at least 24 hours in advance if they cannot attend.
 

7. Emergencies and Crisis Support

a.) Find My Focus is not an emergency service provider.

b.) If you or a loved one is experiencing a crisis, please call 911 or contact a local crisis center.

c.) If you express a threat of self-harm or harm to others during a session, we are legally and ethically obligated to notify emergency services or other appropriate authorities to ensure safety.
 

8. Patient Conduct and Professional Standards

a.) All patients are expected to engage with Find My Focus staff and providers in a respectful, honest, and cooperative manner. Inappropriate, abusive, or disruptive behavior—whether verbal, written, or behavioral—may result in immediate termination of services.

b.) Attempts to deceive or manipulate clinical staff, including falsification of information, misuse of prescriptions, or requests for medications under false pretenses, are strictly prohibited and may be reported to the appropriate licensing and regulatory authorities.

c.) Find My Focus reserves the right to discharge any patient whose conduct compromises the integrity of care delivery, violates professional boundaries, or disrupts the clinical environment. This includes violations of applicable law or these Terms and Conditions.

d.) Patients are expected to actively participate in their treatment, adhere to agreed-upon care plans, and attend scheduled appointments. Repeated non-compliance may result in review or discontinuation of services.
 

9. Referrals and Continuity of Care

a.) Referrals can be made by family members, schools, medical providers, or patients themselves.

b.) All referrals are reviewed for clinical appropriateness before acceptance.

c.) If our care model is not the right fit for your needs, we will assist in referring you to a more appropriate provider or specialist.
 

10. Use of Website Content

a.) The resources, assessments, and blog posts available on the Find My Focus website are for informational and educational purposes only. They are not intended to replace professional clinical evaluation.

b.) Use of these materials does not establish a provider-patient relationship.
